Client's Rights Under HIPPA
- Right to a Notice of Privacy Practices All client receive and sign a client contract at the time of admission that states our confidentiality and privacy policies.
- Right to Access Private Health Information: Patients have the right to get a copy of their Private Health Information (PHI) that are in our records. We require that these requests are made in writing and are subject to a minimum $10 records fee.
- Right to Request to Amendments to PHI: Patients have the right to ask us to amend or correct their PHI. These requests are required in writing, we do not grant all requests.
- Right to Request Alternative Means of Communicating PHI Patients may request that we communicate their PHI by alternative means (fax) or to an alternative location. Requests must be made in writing.
